Hours: M-F 8am-8pm Saturday and Sunday plus 1 additional 7.5 hour shift during the week.

Hourly Rate/Salary: 83k-95k

Job Summary: 

A well-established healthcare organization in the Erie County area is currently seeking passionate and empathetic nurses to present hospice services to patients and families.

• Perform comprehensive assessments to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness that must be addressed in order to promote the patient’s well-being, comfort, and dignity throughout the dying process and documents findings appropriately in the EMR

• Perform pain assessments using, adhering to pain protocols, knowledgeable in equi-analgesic dosing; optimal pain relief using an interdisciplinary focus and timely interventions

• Articulate clinical course/disease progression of cancer(s), lung disease, heart disease, liver disease, dementia, neurologic disease states

• Initiate the plan of care timely and appropriately with a demonstrated skill in anticipating problems that may cause or create a threat to patient safety or lack of symptom control

• Perform other duties, as needed
