Hours: M-F 9:00am-5:00pm - Remote after training onsite in New York, NY

Hourly Rate/Salary: 85k-105k + bonus

 Job Summary: 

A large healthcare organization with a strong reputation in NY is currently seeking Nurse Care Managers to join their team.

Reporting to the Supervisor of Care Management this position will provide case management to the organization's members including medical, social, psychological, physical and spiritual needs by Developing, implementing and monitoring the care plan, in conjunction with the PCP, caregivers and other team members, to help meet the member's needs.

The Care Manager (RN) will:

• Provide case management to members, including medical, social, psychological, and physical.

• Develop, implement, and monitor the care plan to help meet the member's needs.

• Perform other duties, as needed.
